I understand that all materials and knowledge, including audios, videos and written materials are copyrighted by Dr. Nancy Lonsdorf and may not be reproduced, replayed (outside of my private viewing) or distributed without the written consent of Dr. Nancy Lonsdorf.
I will not become a patient of Dr. Nancy Lonsdorf as a result of this program. No doctorpatient relationship is implied or created by my participation in this program. This course is not a replacement for medical advice I receive from my health care provider.
Undergoing any medical testing described in this program is my personal choice and responsibility and not a prescription by Dr. Lonsdorf. I take full responsibility for all consequences, including but not limited to, physical, medical and psychological, of any testing that I undergo arising out of or relating to this program. Undergoing medical testing is not necessary for participation in this educational program about anti-aging and rejuvenation.

Any dispute arising out of or relating to the program will be resolved in the courts of Iowa under the internal laws of the state of Iowa (and not its conflicts of laws principles).
